Job Summary:

We are seeking compassionate and dedicated Direct Care Workers to provide support and assistance to individuals with developmental disabilities, seniors, and individuals requiring memory care. As a Direct Care Worker, you will play a vital role in improving the lives of those in need by offering personalized care and promoting their overall well-being.

### Duties:

- Assist individuals with daily activities such as bathing, dressing, grooming, and meal preparation
- Provide companionship and emotional support to clients
- Administer medications as prescribed by healthcare professionals
- Monitor vital signs and report any changes or concerns to the appropriate personnel
- Assist with mobility and transfers, including the use of lifting equipment if necessary
- Implement care plans and follow established protocols for each client
- Document observations, interventions, and progress notes accurately and timely
- Collaborate with case managers and other healthcare professionals to ensure coordinated care

### Requirements:

- Experience working with individuals with developmental disabilities, seniors, or individuals requiring memory care is preferred
- Knowledge of dementia care principles and techniques is a plus
- Ability to lift and assist with transfers as needed
- Familiarity with case management procedures is beneficial
- First aid certification is required (or willingness to obtain)
- Strong communication skills and the ability to work effectively in a team environment
